630 Outlaws Code of Conduct Form
630 Outlaws YFC will not tolerate verbal abuse of its volunteer coaches from any Fan, Parent or Spectator.
This is American Youth Football, not the pros. Fans, as well as the players and coaches, are expected to abide by a code of conduct at all American Youth Football Events. While 99% of the adults in the program will abide by this code without being told, this code is being published to protect the children and volunteers, from the ones that will not
FANS' CODE OF CONDUCT
Fans will abide by a Code of Conduct which includes the provisions which follow. If any of these rules are broken, 630 Outlaws YFC shall have the authority to impose a penalty.
Fans shall:
1. Not criticize the players/cheerleaders or coaches in front of the other spectators in the stands, but reserve constructive criticism for later, in private.
2. Accept decisions of the game officials (including referees and coaches) on the field as being fair and called to the best ability of said officials.
3. Not criticize an opposing team, its players, coaches, or fans by work of mouth or by gesture.
4. Refrain from using physical or verbal abuse or profane language at any time at the game, practice field, or other AYF functions.
5. Abstain from being under the influence of or in possession of and/or drinking alcoholic beverages and the possession or use of any illegal substance on both the game and practice fields.
6. Not be allowed on the sidelines during a game.
7. Not interfere/interrupt the coaching staff before, during or after games or at practice.
8. Not express complaints about coaches in stands or to coaches in front of or around the children (i.e. right after a game or practice).
VIOLATION
Any parent or fan who violates the code of conduct risks the further participation of the child in the program. The procedure is as follows:
1. Any fan who violates the code of conduct or becomes a nuisance will be asked to leave by the head coach and can be suspended from all team activities. Any parent asked to leave will not be allowed back.
2. If the fan fails to leave upon request, the child may be suspended from further participation in team activities by the head coach.
3. The head coach will decide if the duration of the suspension is to be longer than one to four weeks or if the child will be dropped from the program. That decision will depend on the attitude of the parents.
4. Any parent or fan who violates the code of conduct risks the future participation of his/her children in the program. Depending on the severity of the incident the board of directors may decide to ban future participation in the program for up to three years.
CONDUCT OF ALL PLAYERS - PARENTS
All players are guaranteed 6 plays in each Jamboree, Regular Season or Playoff game. Everything beyond that must be earned in the opinion of the coaching staff whose decisions are final.
Athlete’s Code
I will: emphasis the ideals of sportsmanship, ethical conduct and fair play. Show courtesy to my opponents and officials. Recognize athletic contests are serious educational endeavors. Give complete allegiance to my coaches who are the instructional authority for my team. Discourage fans, fellow players and parents from undercutting my coach’s authority.
I will not: Use profanity or talk “trash” before, during or after any game. Use drugs, alcohol, or tobacco. Criticize my teammates. Act in any way that may incite spectators.
Parent’s Code
I will: Support my child’s team/squad and teach the value of commitment to the team/squad - emphasis the ideals of sportsmanship, ethical conduct and fair play. Help my child and American Youth Football make athletic contests a positive educational experiences. Show courtesy to opponents and officials. Direct constructive criticism of my child’s athletic program to the athletic director or association officials and work toward a positive result for all concerned.
I will not: Criticize officials, direct abuse or profane language toward them, or otherwise subvert their authority. Undermine, in work or deed, the authority of the coach or administration. Intrude onto the field, stand on the sideline, or yell from the bleachers at or to the coaches, referees or administration.
Release of Liability(Required) I have read and agree to the Release of Liability
RELEASE OF LIABILITY AGREEMENT
In consideration of being allowed to participate in exercise programs at 630 Outlaws (“Outlaws”), including the use of equipment and the facilities, I do whereby waive, release and discharge Outlaws and its employees, officers, interns, volunteers, agents, and representatives of any/all responsibility and liability for any injury that I may sustain resulting from my participation in activities, or use of the facility and any/all equipment. For, and in further consideration for my participation in this activity, I hereby agree to hold harmless, the Company, its personnel, and/or its owner(s), from any and all claims, suits, losses or related cause of action for damages incurred during or arising in any way from this activity, on behalf of myself, my family, and my estate. I waive any liability or responsibility or negligence of the Company for the same.
I understand and am aware that strength, flexibility, and aerobic exercise (including the use of Outlaws equipment) is potentially a hazardous activity. I also understand that fitness activities involve a risk of injury and I am voluntarily participating in exercise programs and use of the facility and any/all equipment with the knowledge of dangers involved. I hereby agree to assume and accept any/all risk associated with my activities in the facility and with any/all equipment at Outlaws.
I understand that activities performed at Outlaws involve exercise programs that will place a workload on my lungs, heart, bones, joints, muscles, and connective tissues (my cardiorespiratory and skeletal/muscle systems).
I do hereby further declare myself to be physically sound and suffering from no condition that would prevent me from participating in any Outlaws exercise program. I willfully and consciously state that I have consulted with – and received approval from – a licensed physical to participate in exercise programs, and I know of no health related or other undisclosed reason that I cannot fully participate, therefore should an issue arise with any/all of the matters listed above, I willingly accept all liability and responsibility – including costs associated with treatment and maintenance – should an issue arise where I require medical, psychological, or physical assistance/services.
I acknowledge that I have had a medical physical examination within the last calendar year, and received approval from a licensed physician to participate in exercise programs, therefore I assume all legal and medical responsibility for my participation in Outlaws exercise programs, including the use of the facility and any/all equipment.
NIL Release(Required) I have read and agree to the Image Usage Waiver
I, and any minor whom I register (all of whom are referred to together as “I” or “my”), wish to
voluntarily participate in a (“Outlaws”) event or activity (the “Activity”). I certify that
I have legal authority to agree to this waiver agreement on my own behalf and/or on behalf of any such
minor. I hereby grant the Outlaws (“Releasee”) the exclusive right and authority to copyright, use, and
publish the voice, picture, name, and likeness of myself in relation to my participation in the Activity for
advertising, publicity, promotional, and other purposes. The right shall always belong to the Releasee
and shall survive termination of this agreement. I understand that no compensation shall be paid or
payable for any such right or use.
I HAVE READ THIS PHOTO AND VIDEO RELEASE AGREEMENT, I UNDERSTAND ITS TERMS, AND I
UNDERSTAND THAT I AGREE TO ITS TERMS BY REGISTERING FOR THE ACTIVITY.

Social Media Agreement
Social media can be a useful tool to communicate with teammates, fans, friends, coaches and more. Social
media can also be dangerous if you are not careful. Every picture, link, quote, tweet, status, or post that you
or your friends put online is forever part of your digital footprint. You never know when that will come back to
hurt or help your reputation during the recruiting process, a new job, or other important areas of your life.
Recognizing the above:
I take responsibility for my online profile, including my posts and any photos,
videos or other recordings posted by others in which I appear.
I will not degrade my opponents before, during, or after games.
I will post only positive things about my teammates, coaches, opponents and
officials.
I will use social media to purposefully promote abilities, team, community, and
social values.
I will consider “Is this the me I want you to see?” before I post anything online.
I will ignore any negative comments about me and will not retaliate.
If I see a teammate post something potentially negative online, I will
have a conversation with that teammate. If I do not feel comfortable doing so, I
will talk to the team captain, or a coach.
I am aware that I represent my sport(s), school, team, family and community at
all times, and will do so in a positive manner.
